How does someone access our services?
|Step 1:||Call Zach Pacha, lead clinician at 515-243-5181.|
|Step 2:||A brief phone interview will be conducted to gather basic information, and to determine if the specific issue(s) of the individual correspond with services provided.|
|Step 3:||Once a treatment process is agreed upon, two meetings are scheduled to set up payment of fees. (Eyerly Ball accepts many insurance providers. A sliding fee scale is offered to accommodate qualifying individuals.)|
|Step 4:||An evaluation will be completed to determine an individualized treatment process.|
